Los Comités Hospitalarios de Bioética (CHB) tienen la atribución de emitir sugerencias al personal de salud, ante casos clínicos difíciles que involucran problemas y/o dilemas bioéticos. A nivel mundial, y también en México, se encontró poca literatura relacionada, generando en ocasiones cierto grado de incertidumbre en los profesionales de la salud para su resolución. Objetivo: describir los motivos de presentación de casos clínicos de pacientes ante el CHB en un hospital de segundo nivel, desde junio de 2007 a junio de (...) 2018. Método: estudio observacional, descriptivo, retrospectivo y transversal; la muestra fue por conveniencia y se obtuvieron 48 registros de minutas. Resultados: el motivo de consulta más frecuente ante los CHB fue el apoyo en la decisión para la limitación del esfuerzo terapéutico, con 24 casos (50%). Se identificaron 34 dilemas bioéticos (71%) y 14 problemas bioéticos (29%). Conclusiones: ayudará a los servicios médicos tratantes a identificar e intervenir oportunamente, al basarse en situaciones previas sobre dilemas y/o problemas bioéticos. (shrink)

Las directrices anticipadas aún no se encuentran legisladas en todos los estados de la República mexicana. En algunos de los estados en los que sí se encuentran legisladas, se prohíbe expresamente su validez durante el periodo de gestación. Lo anterior representa dilemas bioéticos y jurídicos importantes, los cuales revisamos para esclarecer las diversas interrogantes que surgen en materia de la protección tanto del bebé como de la madre gestante. Concluimos que existe una necesidad de desarrollar una ley federal que homologue (...) las directrices anticipadas, garantizando una muerte digna a todos los mexicanos, y que establezca que en caso de encontrarse en periodo de gestación, se busque asesoría por un comité de bioética que apoye a determinar en cada caso particular, las condiciones que favorezcan el desarrollo y supervivencia del bebé, siempre que sea posible, sin recurrir a tratamientos y/o medidas extraordinarias que pongan el riesgo la muerte digna y libre de sufrimiento de la madre. There is an ongoing debate about the ethics of research on lifespan extension: roughly, using medical technologies to extend biological human lives beyond the current “natural” limit of about 120 years. At the same time, there is an exploding interest in the use of artificial intelligence (AI) to create “digital twins” of persons, for example by fine-tuning large language models on data specific to particular individuals. In this paper, we consider whether digital twins (or digital doppelgängers, as we refer to (...) them) could be a path toward a kind of life extension—or more precisely, a kind of person extension—that does not rely on biological continuity. We discuss relevant accounts of consciousness and personal identity and argue that digital doppelgängers may at least help us achieve some of the aims or ostensible goods of person-span extension, even if they may not count as literal extensions of our personhood on dominant philosophical accounts. We also consider relational accounts of personhood and discuss how digital doppelgängers may be able to extend personhood in a relational sense, or at least secure some of the goods associated with relevant relationships. We conclude by suggesting that a research program to investigate such issues is relevant to ongoing debates about the ethics of extending the human lifespan. (shrink)

Why should we care what happens to future generations? Samuel Scheffler argues that we are more invested in the fate of our descendants than we may realize. Implicit in our own attachments are powerful reasons for wanting the chain of human generations to persist into the indefinite future under conditions conducive to human flourishing.

This paper concerns the grounds of nonground: what it is in virtue of that facts of the form [F1 does not ground F2] hold. While the literature on iterated ground is expansive, there is comparatively little written on the grounds of nonground. I argue that nonground is grounded in distinctness from ground. If F1 does not ground F2, then [F1 does not ground F2] is grounded in the fact that F1 is distinct from that which does ground F2. While this (...) proposal strikes me as natural, it faces puzzles involving conjunction, contingency, and cardinality. The bulk of this paper explores how the view might be precisified, and how these puzzles might be resolved. (shrink)

End-of-life decision making is fraught with ethical challenges. Withholding or withdrawing life support therapy is widely considered ethical in patients with high treatment burden, poor premorbid status, or significant projected disability even when such treatment is not ?futile.? Whether such withdrawal of therapy in the aftermath of attempted suicide is ethical is not well established in the literature. We provide a clinical vignette and propose criteria under which such withdrawal would be ethical. We suggest that it is appropriate to withdraw (...) life support, regardless of the cause of the critical illness or disability, when the following criteria are met: (1) Surrogates request withdrawal of care and the adequacy of surrogates is confirmed, (2) an external reasonability standard is met, (3) passage of time, perhaps 72 hours, to allow certainty regarding the patient's wishes, and (4) psychiatric morbidity should be considered as grounds for withdrawal only in truly treatment-refractory cases. Fundamentally, we believe the question to ask is, ?If this were not an attempted suicide, would a request to withdraw care be reasonable?? We believe that under these circumstances, such withdrawal of life support, even in an individual who has attempted suicide, does not constitute physician assistance with suicide and is distinct from physician aid-in-dying in several important respects. Purity is the principle that fundamental facts only have fundamental constituents. In recent years, it has played a significant role in metaphysical theorizing—but its logical foundations are underdeveloped. I argue that recent advances in higher-order logic reveal a subtle ambiguity regarding Purity’s interpretation; there are stronger and weaker versions of that principle. The arguments for Purity only support the weaker interpretation, but arguments that employ it only succeed if the stronger interpretation is true. As a result, nearly every metaphysician who (...) has appealed to Purity has made a mistake—in that the inferences that they make are not justified by the arguments that they provide. (shrink)

In Nicholas Smyth’s recent article, “When Does Self-Interest Distort Moral Belief,” he argues that self-interest undermines justification for moral belief if it justifies itself. In so doing, he opposes the standard account, which says that, to the extent that a person’s moral belief is explained by her egoistic or parochial interests, that belief is less justified. However, Smyth’s attack on the standard account, and the principle that he proposes to replace it with, do not withstand critical scrutiny, and that is (...) what this article is intended to show. To that end, the article is divided into three sections. In the first, I explain the standard account and provide some background. In the second, I critically examine Smyth’s three arguments against the standard account. In the third, I explain and attack Smyth’s own principle. I conclude that Smyth’s principle should not be accepted, and the standard account should not be rejected, at least on the basis of Smyth’s arguments: self-interest undermines justification in general, as per the standard account, not only when it justifies itself. He explores how Kantian principles on the dignity of persons shed light on pressing issues in modern bioethics, including the distribution of scarce medical resources and the regulation of markets in organs.

As concern over the use of questionable research practices in academic science has increased over the last couple of decades, some reforms have been implemented and many others have been debated and recommended. While many of these proposals have merit, efforts to improve scientific practices are more likely to succeed when they are responsive to the prevailing views and concerns of scientists themselves. To date, there have been few efforts to solicit wide-ranging input from researchers on the topic of needed (...) reforms. This article is a qualitative report of responses from federally funded scientists to the question of what should be done to address the problem of QRPs in their disciplines. Overall, participants were concerned about how institutional and career-oriented incentives encourage the use of QRPs. Compared to previous recommendations, participants had surprisingly little confidence in the ability of ethics training to improve research integrity. (shrink)

Understanding inductive reasoning is a problem that has engaged mankind for thousands of years. This problem is relevant to a wide range of fields and is integral to the philosophy of science. It has been tackled by many great minds ranging from philosophers to scientists to mathematicians, and more recently computer scientists. In this article we argue the case for Solomonoff Induction, a formal inductive framework which combines algorithmic information theory with the Bayesian framework. Although it achieves excellent theoretical results (...) and is based on solid philosophical foundations, the requisite technical knowledge necessary for understanding this framework has caused it to remain largely unknown and unappreciated in the wider scientific community. The main contribution of this article is to convey Solomonoff induction and its related concepts in a generally accessible form with the aim of bridging this current technical gap. In the process we examine the major historical contributions that have led to the formulation of Solomonoff Induction as well as criticisms of Solomonoff and induction in general. In particular we examine how Solomonoff induction addresses many issues that have plagued other inductive systems, such as the black ravens paradox and the confirmation problem, and compare this approach with other recent approaches. The philosophy of Henri Maldiney has played an important role in the evolution of French philosophy, especially its phenomenological strand. Maldiney's ideas have to a large extent developed from a close study of psychopathology. In this article, I present some of the key principles of Maldineyan thought, which has found little recognition to date in Anglophone philosophy and psychopathology. My main purpose is to explain the psychopathological and therapeutic implications of these principles. First, I make a few observations about Maldiney's (...) life and then sum up some of the main concepts of Maldineyan thought, such as sensing, rhythm, and, most important, openness to the event. This article examines the significance of Jacques Rancière’s work on pedagogy, and argues that to make sense of Rancière’s ‘lesson on the lesson’ one must do more but also less than merely explicate Rancière’s texts. It steadfastly refuses to draw out the lessons of Rancière’s writings in the manner of a series of morals, precepts or rules. Rather, it is committed to thinking through the ‘lessons’ of Rancière in another sense. Above all, Rancière wants to ‘teach’ his readers something absolutely (...) crucial about teaching. In making this claim the article emphasizes the extent to which Rancière advocates an utterly radical pedagogy, one that completely reconceives all the central elements of ‘schooling’, including teacher, student, intelligence and knowledge. Rancière thinks it possible to teach without knowing; he believes that the best schoolmasters can operate not on the assumption of their expertise, but on the equality of intelligence; and this means ultimately that Rancière contends that we can ‘teach what we do not know’. The best schoolmasters are ignorant schoolmasters. Rancière’s radical pedagogy depends upon, just as it consistently advances, a thoroughgoing resistance to a certain form of epistemological and ontological mastery. The rejection of mastery—of schoolmasters who would know it all, and convey this knowing to their students—forms the very backbone of all of Rancière’s writings and critical investigations. This is the chief reason why Rancière is, in a way, always talking about pedagogy, even when his subject matter appears to be something else entirely. (shrink)

Some have argued that advances in the science of human decision-making, particularly research on automaticity and unconscious priming, would ultimately thwart our commonsense understanding of free will and moral responsibility. Do people interpret this research as a threat to their self-understanding as free and responsible agents? We approached this question by seeing how feelings of surprise mediate the relationship between personal sense of control and third-personal attributions of free will and responsibility. Across three studies (_N_ = 1,516) we found that (...) people with a greater sense of personal control were more surprised at the results of experiments showing effects of unconscious priming on moral behavior. Surprise differentially mediated the relationship between personal control and attributions of free will and responsibility: people attributed _less_ free will and _more_ responsibility as they were more surprised. This suggests that people exhibit defensive thinking with respect to responsibility, but not free will. (shrink)

Suppose that a young athlete has just become quadriplegic. He expects to live several more decades, but out of self‐interest he autonomously chooses to engage in physician‐assisted suicide (PAS) or voluntary active euthanasia (VAE). Some of us are unsure whether he or his physician would be acting rightly in ending his life. One basis for such doubt is the notion that persons have dignity in a Kantian sense. This paper probes responses that David Velleman and Frances Kamm have suggested to (...) the question of whether participating in PAS or VAE to benefit oneself, as the young man might, respects the dignity of persons, specified in an orthodox Kantian way. Velleman claims that it does not, while Kamm insists that, in certain circumstances, it does. I argue against Kamm's position. I go on to contend that while orthodox Kantianism might provide a basis for moral concern regarding the case of the young quadriplegic, it suffers from two serious shortcomings. First, it implies that terminally ill patients are wrong to request VAE or engage in PAS to avoid intense suffering, at least when this suffering has not yet overwhelmed their reason. Second, orthodox Kantianism implies that it is wrong for physicians to withdraw such patients from life‐sustaining treatments, even if they request it. To remedy these shortcomings, I sketch an unorthodox Kantian account of respect for the dignity of persons. This account promises to capture the idea that it would be morally problematic for doctors to help the young quadriplegic to die, but to avoid the shortcomings of an orthodox Kantian account. This paper is concerned with counterfactual logic and its implications for the modal status of mathematical claims. It is most directly a response to an ambitious program by Yli-Vakkuri and Hawthorne, who seek to establish that mathematics is committed to its own necessity. I demonstrate that their assumptions collapse the counterfactual conditional into the material conditional. This collapse entails the success of counterfactual strengthening, which is controversial within counterfactual logic, and which has counterexamples within pure and applied mathematics. Partial consistency statements can be expressed as polynomial-size propositional formulas. Frege proof systems have polynomial-size partial self-consistency proofs. Frege proof systems have polynomial-size proofs of partial consistency of extended Frege proof systems if and only if Frege proof systems polynomially simulate extended Frege proof systems. We give a new proof of Reckhow's theorem that any two Frege proof systems p-simulate each other. The proofs depend on polynomial size propositional formulas defining the truth of propositional formulas. These are already known to (...) exist since the Boolean formula value problem is in alternating logarithmic time; this paper presents a proof of this fact based on a construction which is somewhat simpler than the prior proofs of Buss and of Buss-Cook- Gupta-Ramachandran. We study the long-standing open problem of giving$\forall {\rm{\Sigma }}_1^b$separations for fragments of bounded arithmetic in the relativized setting. Rather than considering the usual fragments defined by the amount of induction they allow, we study Jeřábek’s theories for approximate counting and their subtheories. We show that the$\forall {\rm{\Sigma }}_1^b$Herbrandized ordering principle is unprovable in a fragment of bounded arithmetic that includes the injective weak pigeonhole principle for polynomial time functions, and also in a fragment that includes the surjective weak pigeonhole (...) principle for FPNPfunctions. We further give new propositional translations, in terms of random resolution refutations, for the consequences of$T_2^1$augmented with the surjective weak pigeonhole principle for polynomial time functions. According to Hendricks Impairment Argument (IA), abortion is immoral because it impairs the fetus. Here I argue it is not sufficient to show merely that abortion impairs, Hendricks must show that it harms the fetus. If the fetus is not numerically identical to the person it will become, then it isn’t harmed by an abortion. But if the fetus is numerically identical to the person it will become, it is harmed by the deprivation of a future of value. However, taking (...) this route ultimately undermines the novelty of the impairment argument. (shrink)

The recent global economic collapse brings new calls for reform and change as well as a re-examination of the ethical foundations underpining it. Most professors as well as students remain profoundly unhappy with the Business Curricula. The curricula appear to swing between technological training and academic theory. There is little genuine focus on the central issue of the problem: the students’ and faculty’s assumptive world which drives the selection of the materials chosen for presentation as well as the decision-making process. (...) In the pragmatic quest to achieve status within academe, business schools appear to have forgotten that their subject matter is not one cognate domain but a mixture of several areas including mathematics, economics, anthropology, sociology, psychology, logic and planning. Course structures must be redesigned as consilient. That is, each course contains in it the links to other courses and is not expected to be complete in themselves; the new structures proposed are no longer under the direct control of one instructor but each course is under the control of a committee. This creates a linkage between courses that together from a linked chain of knowledge where the strength of the curriculum is tied to the consilient strength of the courses. The result is an organic and developmental model for teaching and learning with a strong ethical foundation as well as developed moral links to effective decision making. (shrink)

This paper studies the developments in the ethical concerns for physicians (Business Concerns) and job satisfaction contigent upon changes in Physicians’ assumptive world.

In this paper I am going to raise a problem for recent attempts to derive positive duties from Kant’s universalizability tests. In particular, I argue that these recent attempts are subject to reductio and that the most obvious way of patching them renders them impracticable. I begin by explaining the motivation for these attempts. Then I describe how they work and begin my attack. I conclude by considering some patches.

Leibniz is an eclectic and ecumenical philosopher. He often worked out philosophical positions that reconciled seemingly opposed theoretical systems and chastised people for rejecting certain views too quickly. In this paper, I describe one episode of Leibnizian reconciliation. My target is the phenomenon of deliberation. Traditionally, philosophers have offered two different accounts of deliberation based on two different accounts of the compatibility of freedom and determinism. Leibniz, I argue, cannot accept either account because of his broader theoretical commitments. This leads (...) Leibniz to formulate an interesting account of deliberation that adopts certain elements from each account while excising the untoward aspects of each one. I outline the various mechanisms involved in deliberation on Leibniz’s view and show how deliberation fits into a broader theory of free action. I close with an assessment of whether Leibniz in fact succeeds in offering an account of deliberation that is distinct from the traditional positions. (shrink)
